The Wellness Center Terms of Service

You assume full responsibility for and agree to pay all costs, charges and expenses of every kind and description for services furnished by The Wellness Center. You agree to pay charges and services not covered by any insurance or other third-party pay and/or not paid to The Wellness Center for any reason with a period of time The Wellness Center deems reasonable.  The amount of the bill shall be due and payable upon presentation to the patient, his/her agent, guardian, conservator or third party responsible for payment of the charges. There are no formal requirements stipulated for massage therapists related to charting/note taking under the Affordable Care Act.  However, I understand that if a confidential record is kept, it will not be released to others unless they are involved in my care plan.

The Wellness Center is not responsible for any condition of which we are not informed, which may worsen over time through treatment.

Our Cancellation Policy is as follows:

For most appointments, we require 4 hours notice. However, all morning appointments (scheduled for before 12:00 PM) require notice by 6pm the day before the scheduled appointment.  Although a courtesy call, email or text is usually made, it is your responsibility to remember and show up for your appointments.  Late cancellations and no show appointments will be charged for the full amount of the service.
